Transitional Cardiology

Transitional cardiology is a brand new field that bridges the gap between hospital based care and skilled nursing facilities as well cardiac care at patient’s own home.  Dr. Junaid Ahmed and staff at Cardiac Vision is the first cardiac practice in the nation to offer these services.

Cardiac Vision provides onsite cardiology in the skilled nursing facility and 24/7 coverage. Aside from having an onsite cardiology provider at each facility, Cardiac Vision also provides

  • Education and training for the nursing staff, especially if there are hospital protocols that need to be implemented. Also, training on managing transplant/LVAD or end stage heart failure patients
  •  Nursing staff will be cardiac certified to manage cardiac patients at Skilled or Assisted Living Facilities
  • EKG’s onsite and training to the staff
  • Real time telemetry
  • Real time event monitors
  • Real time AICD/Pacemaker checks
  •  Echocardiography onsite

 

 

Mostly the elderly who suffer from a variety of health conditions often need health care services in different settings to meet their many needs. Transitional cardiac care is based on a comprehensive plan of care that is provided by our physicians and health care practitioners who are well-trained in chronic care to meet the patient’s individualized goals, preferences, and clinical status. It includes logistical arrangements, education of the patient and family, and coordination among the patient’s own cardiology group.  

In the Jacksonville location, Cardiac Vision has partnered up with all of St. Vincent’s Hospital’s cardiology groups, Baptist cardiology groups, UF Health, as well as Orange Park Cardiology groups to make the smooth transition from the hospital to the SNF, rehab, or LTAC setting. Furthermore, we also provide cardiac care at patients home for those that are bed bound via our Cardiac Vision house call practice. Transitional cardiac care, which encompasses the transition from hospital discharge to patient’s own cardiologist, is essential for all patients with complex cardiac care needs.

Renal diseases consists of a vast spectrum of conditions. Our team is focused on prevention, diagnosis and treatment of these complex disorders.

It includes logistical arrangements, education of the patient and family, and coordination among the patient’s own cardiology and Nephrology groups.

Make sure All patients return to their outpatient Cardiologist or Nephrologist.
